Understanding what your MRI really shows

The Key Difference

Many patients are confused when their MRI shows disc problems. Understanding the difference between a herniated disc and degenerative changes is important:

Herniated disc: This can happen suddenly when the inner disc material ruptures through the outer disc wall, often from injury or strain. It may press on nerves causing immediate severe symptoms.

Degenerative disc disease: This is a gradual process of normal aging where discs lose water content, height, and flexibility over many years. It's extremely common and often causes no symptoms.

How Common Are These Findings?

Research on people WITHOUT back pain shows:

  • 30% of 20-year-olds have disc bulges on MRI
  • 40% of 30-year-olds have disc degeneration
  • 60% of 40-year-olds show degenerative changes
  • 80% of 50-year-olds have disc degeneration
  • 90% of 60-year-olds show degenerative changes

Important: These findings are in people who have NO pain. Having degeneration on MRI doesn't necessarily mean that's the cause of your pain.

Signs of a Herniated Disc

Symptoms often include:

  • Sudden onset of severe pain, often after lifting or bending
  • Sharp pain radiating down leg (sciatica) or arm
  • Numbness or tingling in specific areas
  • Weakness in affected limb
  • Pain worse with sitting, bending, coughing

On MRI:

  • Disc material extending beyond normal boundaries
  • Nerve compression visible
  • May show "high-intensity zone" indicating tear

Signs of Degenerative Disc Disease

Symptoms often include:

  • Gradual onset of aching back or neck pain
  • Pain worse at end of day or after activity
  • Stiffness in the morning
  • May have periods of flare-ups and improvement

On MRI:

  • Dark discs (indicating water loss)
  • Loss of disc height
  • Bone spurs (osteophytes)
  • Facet joint arthritis
  • Changes in endplates (Modic changes)

Can Both Happen Together?

Yes, absolutely. You can have age-related degeneration AND develop an acute herniation. In fact, degenerative discs are more prone to herniation because the outer disc wall weakens over time.
